Salem Health Clinics is seeking a dedicated Family Medicine Physician Associate or Nurse Practitioner to provide comprehensive primary care to patients in the heart of Oregon’s Willamette Valley. Monmouth Family Medicine Clinic
Salem Health’s Monmouth Family Medicine Clinic is seeking an accomplished Nurse Practitioner or Physician Associate to join our dedicated care team. Our current group includes one Family Medicine Physician, and two Nurse Practitioners. You will manage a full patient panel with a schedule that remains flexible and adjustable based on patient and clinic needs. As the only primary care providers serving a community of approximately 10,000 residents, our clinicians play a critical role in ensuring access to comprehensive, high-quality medical services. Providers also benefit from being a part of Salem Health Medical Group, a strong network of more than 250 clinicians across 16 specialties, all committed to delivering exceptional patient experiences and evidence-based care.
What you'll do
- Provide comprehensive primary care, including wellness exams, chronic disease management, and acute care.
- Obtain and document patient histories, perform physical exams, and establish treatment plans.
- Order and interpret diagnostic tests, prescribe medications, and perform minor procedures.
- Educate patients and families on health conditions, prevention, and treatment options.
- Collaborate with physicians and care team members to ensure coordinated, high-quality care.
- Participate in clinic initiatives, protocol development, and professional development activities.
Required qualifications
Nurse Practitioner (NP):
- Master’s degree or higher in Nursing or equivalent field recognized by the Oregon Board of Nursing.
- Completion of a Nurse Practitioner program recognized by the Oregon Board of Nursing.
- Current, unencumbered RN license and NP certificate in the State of Oregon.
- Valid NP national certification and DEA registration.
- At least 12 months relevant hospital experience in the past three years.
- If the Nurse Practitioner does not have the relevant hospital experience and cannot otherwise document current competency, a monitoring plan may be required.
- Current American Heart Association ACLS certification.
Physician Associate (PA):
- Graduation from an ARC-PA–accredited Physician Associate program.
- At least 12 months relevant inpatient experience in the past three years.
- If the Physician Associate does not have the relevant hospital experience and cannot otherwise document current competency, a monitoring plan may be required.
- Successful completion of the Physician Associate National Certifying Exam (PANCE).
- Current certification by NCCPA, Oregon PA license, and DEA registration.
