Your role with us
As a pharmacist with Salem Health, you will play a vital role in supporting safe, effective, and patient-centered medication therapy across our ambulatory care settings. In this flexible, as-needed role, you may be called upon to support variable shifts, extended coverage, special projects, or unexpected staffing needs. You’ll collaborate closely with providers, nurses, and care teams while supervising pharmacy support staff and ensuring the accurate preparation and dispensing of medications in alignment with regulatory standards and Salem Health policies..
What you'll do
- Review and evaluate patient electronic medical records, including medical history, clinical and laboratory data, and current medications (including OTC and home therapies).
- Evaluate and verify provider medication orders for appropriate drug selection, dosage, route, stability, and potential interactions; recommend therapy changes as appropriate
- Monitor and manage medication therapy for assigned ambulatory (and occasional inpatient, as needed) patients to ensure optimal, cost-effective outcomes
- Communicate with prescribing providers to resolve medication-related issues and document therapeutic interventions..
- Provide medication information, drug therapy recommendations, and pharmacokinetic dosing guidance to medical staff.
- Order and adjust medication dosages, order laboratory tests, and administer medications under protocol in accordance with established policies.
- Supervises and directs activities of certified pharmacy technicians, pharmacy interns and pharmacy students as well as pharmacy residents who assist the pharmacist in the practice of pharmacy.
- Verify accuracy of medication preparation and dispensing performed by pharmacy personnel.
- Serve as a liaison to assigned departments or clinics, supporting medication inventory management, par levels, and required inspections.
- Participate in special projects, extended coverage, or unexpected staffing needs as required in an unscheduled capacity.
Required qualifications
- Doctor of Pharmacy (PharmD); Bachelor of Pharmacy accepted in lieu of PharmD.
- Active Oregon Pharmacist License.
- Basic Life Support (BLS) certification within two months of hire.
Additional information
- Pharmacy Residency experience preferred.
