Your role with us
This pharmacist opportunity at Salem Health centers on IV room and inpatient pharmacy practice within a 7-on-7-off schedule and plays a key role in ensuring safe and effective medication therapy through order verification, sterile compounding oversight and direct collaboration with physicians and nursing teams. This role is a great fit for a clinically focused pharmacist who can manage complex inpatient regimens, supervise pharmacy team members and serve as a resource during unit care rounds. The ideal candidate will be a detail-oriented problem-solver with strong communication skills and experience in sterile compounding and hospital pharmacy operations
What you'll do
- Review patient electronic medical records including history, clinical and laboratory data and concurrent medications and use pharmacy technician interview summaries to assess medication use, allergies and sensitivities.
- Participate in unit care rounds and collaborate with the care team to provide patient-specific recommendations.
- Verify provider orders for drug, dose, interactions, route, stability and compatibility and recommend therapy changes as appropriate.
- Monitor and manage ongoing medication therapy for assigned inpatients and ambulatory patients to achieve optimal outcomes in a cost-effective manner.
- Communicate with prescribers to resolve issues, document interventions and ensure accurate and timely medication distribution while providing drug information, therapy options and pharmacokinetic dosing guidance.
- Order or adjust dosages, laboratory tests and medication administration under approved protocols.
- Supervise certified pharmacy technicians, interns, students and residents and verify accuracy of medication preparation, dispensing, delivery and administration.
- Serve as liaison to nursing units, maintain appropriate inventory and par levels, complete monthly inspections, deliver education to staff and patients, participate in committees and quality programs and maintain clinical competence through continuing education as required by the State of Oregon.
Required qualifications
- Doctor of pharmacy degree or a bachelor of science in pharmacy.
- State of Oregon pharmacist licensure.
- Basic Life Support (BLS) certification obtained within two months of hire.
- Advanced Cardiovascular Life Support (ACLS) certification obtained within two months of hire.
