Your Role With Us:

  • Performs an accurate, ongoing, pertinent assessment for each assigned patient that are age appropriate. Reports/consults pertinent data to other health team members and implements appropriate action.
  • Assesses historical data pertinent to medical, social, cultural and educational factors and prior rehabilitation efforts; All areas of the evaluation are addressed.
  • Demonstrates competency in delivery of care based on the patient care plan. Performs nursing skills following Salem Health-West Valley Hospital policies and procedures utilizing appropriate resources. Demonstrates knowledge of age-specific behaviors when caring for patients.
  • Initiates and/or reviews appropriate plan of care with patient/family to address holistic age-specific needs and reflect the patient’s changing condition; develops a plan of treatment based on the patient’s rehabilitation potential, rehabilitation barriers, objective findings, cultural, social, medical and developmental backgrounds.
  • Consistently documents the assessment, planning, interventions and patient responses in a way that accurately reflects the patient’s care and progress. (Admission Assessment, medication management, pain assessment and management, vaccination status, core measures, patient education, plan of care initiation and updates, discharge documentation, etc.)
  • Consistently utilizes good communication skills; Communicates accurate and complete information in a clear and concise manner.
  • Seeks and utilizes evidence based practice; Actively engaged in continuous professional development.
  • Assists in maintaining a safe unit environment for patients, staff, and self, i.e. replacing SHARP’s containers, following infection control policies, taking broken equipment out of service, using proper lifting and transfer techniques, keeping the hallways free of clutter/spills, etc.
  • Demonstrates ability and willingness to assist management staff in full utilization of their skills. Flexibility, cooperative effort, and time management are demonstrated on a continual basis through a willingness to be cross-trained to other areas of service.

    Background/Expertise:

    • Bachelor's Degree in Nursing (BSN) required; OR Associate Degree of Nursing (ADN) Graduate of a Diploma RN Program and at least 5 years of acute care RN experience ADN;  OR non-nursing bachelor’s degree with MSN, or MSN; preferred for West Valley Hospital. ADN graduate with less than 5 years of acute care experience required to sign agreement to complete BSN or MSN within 3 years of hire. Note: Employees hired as an ADN prior to October 2011 are not required to obtain BSN or MSN.
    • Minimum of one (1) year of RN Staff Nurse experience required, acute care RN Staff Nurse experience preferred.
    • Current unencumbered licensure to practice nursing as a Registered Nurse in the State of Oregon required.
    • BLS issued by the American Heart Association required.
    • Knowledge of medical terminology and basic clinical skills.
    • Ability to work effectively under pressure and to use professional judgment in emergency situations.
    • Must have basic computer knowledge and keyboarding skills; Ability to collaborate and work in a team environment.
    • Ability to communicate clearly and constructively-written and verbal. (Legible handwriting, accurate spelling, clear enunciation, well modulated voice).
