Your Role With Us:
- Provide expert individualized care coordination to cancer patients and their families including; Provide effective orientation to cancer care delivery system to newly diagnosed patients.
- Provides diagnosis and treatment education to patients and families; Assess patient and identify patient needs utilizing the comprehensive needs assessment.
- Communicate with oncology practitioners, and coordinate plan of care according to established practice guidelines and standards that addresses patient need(s); Provide explanations to patients and families regarding expectations and plan of care.
- Contact patients at high-stress points and connect with resources and support systems; Problem-solve issues to ease patient transitions through the system.
- Facilitate access for referrals, social services, dietitian, physical therapy, testing, procedures, and results; Address financial and community resources.
- Manage content of evidence based patient/family educational curriculum; provides individual and group/family education.
- Serve as link between patient, physicians’ office and Salem Health across the patient care experience.
- Provides psychosocial and emotional support to patients and family; Understands other resources/services; hospice, palliative care, survivorship etc.
- Maintain general and current knowledge of cancer diagnosis, treatments, follow-up, and survivorship recommendations; Serves as program representative and spokesperson in the community.
- Establish expertise in tumor site as established by department need; Participate in Tumor Site Committee(s) as a member of the multi-disciplinary team and as a representative of Cancer Support Services.
- Supports Commission on Cancer and NAPBC accreditations through participation in Tumor Site Committees, Cancer Committee, and service line improvement projects.
- Develop and regularly provide performance tracking and quality improvement recommendations; monitor process and patient outcome data as well patient experience tracking.

Background/Expertise:
- Requires BSN or non-nursing Bachelor’s Degree with MSN.
- Oncology Certified Nurse (OCN) or Certified Breast Care Nurse (CBCN) preferred upon hire or required to achieve within 2 years of hire/or equivalent for part time position.
- Licensed/Registered Nurse in state of Oregon required.
- Basic Life Support within 90 days of hire required.
- Minimum of one (1) year of acute care RN Staff Nurse or Clinic experience required.
- Oncology experience highly preferred; Experience with oncology-related care coordination/case management strongly preferred.
- Case management/navigation and/or coordination experience.
- Ability to work collaboratively with patients, physicians, hospital staff, office staff, volunteers, and community groups to promote optimal patient experience and utilization of resources.
